What are the benefits and adverse effects of HRT on the cardiovascular system, on the brain, on the risk for breast cancer, on the risk for all other cancers, and on the risk for blood clotting? All oral forms of estrogen are associated with blood clotting. Transdermal forms of estrogen are not associated with blood clots. Thus, transdermal estrogen is preferable to oral estrogen. The safest estrogens reduce the risk of breast cancer and are those that target ER beta, the tumor suppressor receptor, such as unsaturated B-ring steroids found in Premarin (CEE), estriol (E3), and testosterone metabolite 3-beta-diol. Exposure to pregnancy levels of hormones confers protection from breast cancer. Long-term estrogen deprivation (LTED) transforms estrogen from a growth factor to a death factor in breast cancer cells.

The exact hormone formula is important for optimal results. For example, a formula using Premarin-alone is not safe because of increased risk for endometrial cancer. Synthetic progestins such as MPA are are endocrine disrupting chemicals which increase risk of breast cancer. In 2011, Dr. Cynthia L. Bethea writes: "While the WHI trial [2002, first arm] made a valuable contribution in revealing the risks associated with conjugated equine estrogens treatment in postmenopausal women, it unfortunately generated considerable controversy in the field because it was interpreted as an indictment of postmenopausal hormone replacement, when in fact, it did not study hormone replacement at all: that would have required use of the natural hormones, estradiol and progesterone. The actions of the natural hormones are significantly different from those of Premarin and MPA."

The 2004 second arm Women's Health Initiative 18 year follow up shows the estrogen treated women (Premarin-alone) enjoy a 45 percent reduction in mortality from breast cancer. This is a paradigm shift in our thinking about hormones and breast cancer. Estrogen is breast cancer protective. In 2022, Dr. Isaac Manyonda writes: "The evidence now compels a paradigm shift from the traditional thinking that estrogen could cause breast cancer to a recognition that it actually prevents the disease, and that when the disease does occur (no preventative intervention achieves a 100% preventative effect), it is often picked up early and mortality is reduced by up to 44%."
