About this listen
Still Not Safe is the story of the rise of the patient - safety movement - and how an "epidemic" of medical errors was derived from a reality that didn't support such a characterization.
Physician Robert Wears and organizational theorist Kathleen Sutcliffe trace the origins of patient safety to the emergence of market trends that challenged the place of doctors in the larger medical ecosystem: the rise in medical litigation and physicians' aversion to risk; institutional changes in the organization and control of healthcare; and a bureaucratic movement to "rationalize" medical practice-to make a hospital run like a factory.
If these social factors challenged the place of practitioners, then the patient-safety movement provided a means for readjustment. In spite of relatively constant rates of medical errors in the preceding decades, the "epidemic" was announced in 1999 with the publication of the Institute of Medicine report To Err Is Human; the reforms that followed came to be dominated by the very professions it set out to reform.

It is hard to change almost a lifetime of your organization’s thinking about safety in the traditional way and it is really difficult to change your organization’s history of bureaucracy that continues to reinforce the traditional safety definitions and metrics our organizations have used for many years. Let’s face it, change even for the better is hard to do. Doing Safety Differently is a discussion between two friends on what they have learned by watching organizations around the world change the way they do work.

Story
We used to think of failure as the opposite of success. Now, we’re often torn between two “failure cultures”: one that says to avoid failure at all costs, the other that says fail fast, fail often. The trouble is that both approaches lack the crucial distinctions to help us separate good failure from bad. As a result, we miss the opportunity to fail well. After decades of award-winning research, Amy Edmondson is here to upend our understanding of failure and make it work for us. In Right Kind of Wrong, Edmondson provides the framework to think, discuss, and practice failure wisely.

The Fearless Organization offers practical guidance for teams and organizations who are serious about success in the modern economy. With so much riding on innovation, creativity, and spark, it is essential to attract and retain quality talent, but what good does this talent do if no one is able to speak their mind? The traditional culture of "fitting in" and "going along" spells doom in the knowledge economy. Success requires a continuous influx of new ideas, new challenges, and critical thought, and the interpersonal climate must not suppress, silence, ridicule, or intimidate.

Why, despite the many advances in science and technology over the past few decades, does our health only seem to be getting worse? Why, despite so much time and energy spent studying the foods we eat, are we more confused than ever about nutrition - what good nutrition looks like, and what it can do for our health? The Future of Nutrition offers a fascinating deep-dive behind the curtain of the field of nutrition - with implications both for our health and for the practice of science itself.

Now, in Power and Prediction, they go deeper, examining the most basic unit of analysis: the decision. The authors explain that the two key decision-making ingredients are prediction and judgment, and we perform both together in our minds, often without realizing it.

Story
Americans have traditionally placed great value on self-reliance and fortitude. Recent decades, however, have seen the rise of a therapeutic ethic that views Americans as emotionally underdeveloped, requiring the ministrations of mental-health professionals to cope with life's vicissitudes. Today, having a book for every ailment, a counselor for every crisis, a lawsuit for every grievance, and a TV show for every problem degrades one's native ability to cope with life's challenges.

Story
The intelligence failures surrounding the invasion of Iraq dramatically illustrate the necessity of developing standards for evaluating expert opinion. This audiobook fills that need. Here, Philip E. Tetlock explores what constitutes good judgment in predicting future events, and looks at why experts are often wrong in their forecasts. Tetlock first discusses arguments about whether the world is too complex for people to find the tools to understand political phenomena, let alone predict the future.

Among the best healthcare books I’ve read
This book connected a lot of disparate threads I’d recognized previously but never put together. In doing so, it helped me better understand the ongoing safety problem in healthcare and what real progress here might look like.
